一、事故经过:

XXXX年X月X日上午,石灰石堆料机大车行走传动轴断裂。

1、两个传动轴都在其输出端轴肩根部全部断裂,断口因断后两断节的相对运动已磨合,不能清晰反映断口表面的宏观组织。

2、传动轴上的两个行走轮侧挡边啃轨较严重,被啃面为挤裂痕迹,两轮被啃面均位于同侧(东侧),磨控痕迹显示轨道与该行走轮长期在此位置运行,因为另一侧轮面和挡边没有接触痕迹。

3、传动轴上的两个行走轮东侧挡边啃轨的同时,东侧3个挡轮(位于啃轨行走轮附近)紧贴导轨侧面,并未发挥作用。

4、行走大车的4对行走轮和4对挡轮相互之间和各自之间的位置关系没有规律比较紊乱,各自不能发挥其正常的作用。

5、直到轴断电机也没有过载报警。

二、事故主要原因:

1、针对上述现场情况并结合设计结构分析,认为主要问题是严重的啃轨现象导致过载断轴。

2、挡轮没有较好地起到挡轮作用,否则可以避免行走轮侧面啃轨现象发生。

三、事故教训及今后的防范措施:

1、按照要求调整行走轮和挡轮的位置。

2、对断轴进行材质和性能分析(如检测硬度,金相组织,判断热处理质量等)。

3、安装减速机时,要先将锁紧盘锁紧装好,此时减速机法兰盘和箱体之间出现周向均匀或不均匀的间隙,该间隙用钢性垫片垫好,然后紧固减速机,注意法兰盘和箱体之间的间隙不得变化。
